REQUIREMENT FOR CHILDREN AND YOUTH SACRAMENT PREPARATION
Sacrament Preparartion is two consecutive years of preparation. Your child is eligible to sign up for the Sacrament Preparation class AFTER your child has already completed one full year of Faith Formation the previous year.
If this is your first year of Faith Formation, use the online Faith Formation Registration form to join our program. The kindergarten program is NOT considered as the first year of preparation for Sacrament preparation.
If Faith Formation was completed from another parish for the first year, we need for you to provide a letter from the director/coordinator of that parish before we can process your registration form.
You will also need to supply us a copy of the child's baptismal certificate BEFORE we can process your registration form as complete.
A completed Confirmation Sponsor form is also required before class begins on September 11, 2022.
If 2022-2023 is your child's second year of Faith Formation, do join our Sacramental Preparation program by filling out this form and making a payment.
Note: In joining our program its a parent's commitment to grow their children in the Catholic Faith. Hence, you are agreeing that your child will continue to be a part of the formation program even after receiving the Sacraments. Holy Family is a " restored order" parish wherein the celebration of the sacraments of Confirmation and First Eucharist are received together. “Restored Order” is to celebrating the sacraments of initiation in the order in which God designed them to be administered: Baptism, Confirmation, and First Eucharist. It also restores the practice of administering the sacraments of Confirmation and First Eucharist to children of early age, which is how the early Church practiced the first 500 years. REQUIREMENT FOR ADULT CONFIRMATION
If you are a Baptized practicing Catholic who never received only the Sacrament of Confirmation when you were younger, classes will be offered in the late Fall of 2022.
All candidates will also need a fully initiated Catholic to be a Sponsor. Let us know if you need help in finding one.
You will need to provide a copy of your Baptismal and First Holy Communion certificates along with your registration form to complete your enrollment.

REGISTER NOW! Registration for the Children's Sacrament Preparation is open and will close on September 11, 2022. After September 11, 2022 we will no longer accept late registration forms.
Sacrament Preparation classes are held from 11 am - 12 noon on most Sundays during the school year. A mandatory Family Orientation Day is on August 28, 2022 for all registered children. The First Day of class will begin on September 11, 2022.
