The priest will help you with a self-testimony (form A) on the first meeting with him.
A parent or someone who knows you very well, is asked to give a witness testimony (form B) before a priest or pastoral minister at Holy Family or in their local parish.
Catholics are to supply a newly issued certificate of baptism with notations. This certificate should be issued by the place of baptism not more than six months before it is presented to the priest who is preparing you for marriage. The new baptism certificate will also list other sacraments you have received. Proof of baptism for a baptized non-Catholic may also be requested. If one of the parties is a non-Catholic Christian or a non-Christian, the priest will help the Catholic party to obtain a permission to enter into a mixed marriage or a dispensation from the disparity of cult.
If you were previously married, you are only free to marry if your previous spouse has died or a declaration of nullity was obtained from the Church. You must inform the priest if you were married before, even in a civil ceremony.
Closer to the time of the wedding, you will have to apply for a marriage license. This can be obtained at the Santa Clara County Clerks Office in San Jose, CA. The license is valid for ninety days. You may contact the Office of the County Clerk for information about fees, blood tests, etc. If you are celebrating marriage in another state, you must be sure to fulfill all of the local requirements, which may differ from one jurisdiction to another. If you are already civilly married, you just provide a certified copy of your marriage certificate to the priest.
